Forgot Password? Click here to reset your password. You will receive an email with a temporary password to use to log into the License Portal. You will then be required to set up your three security questions/answers and to reset your password. Remember, your username, password and answers to the three security questions are all CASE-SENSITIVE.
After three failed login attempts, you will be locked out of Application Portal. If this happens you will not receive a temporary password so you will need to contact the LBVM Office at firstname.lastname@example.org to have your account unlocked.
