Forgot Password? Click here to reset your password. You will receive an email with a temporary password to use to log into the License Portal. You will then be required to set up your three security questions/answers and to reset your password. Remember, your username, password and answers to the three security questions are all CASE-SENSITIVE.

After three failed login attempts, you will be locked out of Application Portal. You will receive an email notification after three failed login attempts with instructions on how to reset your login credentials.

The LBVM Application Portal has recently undergone an upgrade, with the new system being launched March 25, 2025. If you previously had an application started or submitted with the LBVM, you must reset your login credentials by clicking on the “Reset Password” link and use the email address previously on record with the LBVM.
