License Directory Request

The Louisiana Board of Veterinary Medicine is allowed by statute to provide a directory of licensees and certificate holders to include public information only. An invoice will be emailed to you and can be paid via credit card through Authorize.net, our payment processor. The directory will be emailed to you in a MS Excel file format once the invoice has been paid in full.

Cost of Directory

The directory cost is $0.02 per licensee included in directory PLUS a nominal credit card processing fee (2.8% + $0.25). The exact total for the requested directory cannot be provided until the form is completed and the directory is compiled based on the data that is being requested.

Processing Time

The turnaround time is typically 1–2 business days for the invoice to be generated and sent. Once payment is received, please allow an additional 1–2 business days for the directory to be provided to the requester.

Is Disciplinary Action Included?

Disciplinary information is not part of the directory and is only available as a Public Records Request for an individual licensee.

DEA and CDS registration details (i.e. – active/expired status, etc) cannot be confirmed by the LBVM and is only available through the DEA and LA Board of Pharmacy, respectively.

Business details (if available) are only that which the licensee indicates is current place of employment.
