Application for LEAD CAET Certification


The CAET who has been authorized by the animal control facility of employment to hold the LBP-CDS and DEA drug registration to purchase Sodium Pentobarbital for the animal control facility MUST apply to the LA Board of Veterinary Medicine for “Lead” designation of his/her Louisiana CAET certificate if there is no licensed veterinarian at the facility. If there is a licensed veterinarian at the facility, then there should NOT be a Lead CAET.

If you currently have an active Full CAET certification with the Board and wish to apply for Lead CAET certification, please follow the steps below:


Step 1. Complete LEAD CAET Application

  1. Log into the LBVM Application Portal and click “Start New Application” on the welcome page. Scroll down and click “Apply” for the the Lead CAET application under “Animal Euthanasia Technician”.
  2. Read the instructions carefully. You can stop at any step and return later to finish the application.
  3. Follow the prompts and instructions for each section of the application, clicking “Save & Continue” after each one.

To expedite processing, skip the Supporting Documents step and submit the application with payment. Supporting documents may be uploaded later. (See step 2 below.)

There are no fees associated with applying for Lead CAET certification.

Application In-Progress or Submitted?

If you have already started the online application process for Lead CAET certification, click below to go to the LBVM Application Portal. Through the portal you can complete and submit your application and supplemental documentation and check on the status of your documents. Applicants are encouraged to frequently log into the LBVM Application Portal for application updates.

Are You Locked Out? After three failed login attempts, you will be locked out of Application Portal. If so, you must email the Board at admin@lsbvm.org for further assistance.


Step 2. Submit All Required Documentation

All required documentation should be emailed to admin@lsbvm.org.

Required Documentation:

The following documents must be uploaded to complete the online application:

  • Submit a copy of your current, active CDS drug license from the LA Board of Pharmacy indicating your Schedule II-N drug status.
  • Submit a copy of the shelter’s current DEA drug registration with the current Lead CAET’s name whom you will be replacing.
  • Submit proof of completion certificate for chemical capture training approved by the Board.

Step 3. Lead CAET Certification Issued… Now What?

Only after you are issued Lead CAET status will you complete the following:

  1. Download your updated CAET Certification in the LBVM License Portal.
  2. Contact the LA Board of Pharmacy (225-925-6496; info@pharmacy.la.gov) to have your status verified and your CDS drug license amended to include the Schedule II-N drugs AND Schedule III-N drugs, giving you access to pentobarbital, ketamine, and telazol.
  3. Contact the DEA to request that the shelter’s DEA license change to include your name on the license and to include Schedule III-N drugs (if it does not already).

Final Document Submission:

Email copies of the following to the Board at admin@lsbvm.org:

  • Your updated CDS license with Schedule II-N AND Schedule III-N status.
  • The shelter’s DEA license updated with your name on the license.

FREQUENTLY ASKED QUESTIONS

Will I Receive a New Certificate Number?

No. While your certificate type will change from Full CAET to Lead CAET, your certificate number will remain the same. The only change will be the addition of “-L” at the end of your certificate number to indicate Lead CAET status.

For How Long is an Application Valid?

In accordance with LAC 46LXXXV.1201.F, an application is valid for one year from the submission date. If all requirements are not met and a certificate is not issued within that time, the application expires and the process – including all fees and required documents – must be completed again.

Criminal Conviction Considerations

Statute §1518 of the LA Veterinary Practice Act establishes that the Board shall have the power to examine and determine the qualifications and fitness of applicants for a license to practice veterinary medicine in the state of Louisiana. In so doing, criminal convictions of an applicant may be used for a basis for denial. In accordance with LA Revised Statute 37:2950, if the conviction directly relates to the practice of veterinary medicine, the Board shall consider all of the following: a) the nature and seriousness of the offense; b) the nature of the specific duties and responsibilities for which the license, permit, or certificate is required; c) the amount of time that has passed since the conviction; d) facts relevant to the circumstances of the offense, including any aggravating or mitigating circumstances or social conditions surrounding the commission of the offense; and e) evidence of rehabilitation or treatment undertaken by the person since the conviction.

In accordance with LA Revised Statute 37:33, an applicant – or a potential applicant – who has a criminal conviction can apply for a statement from the Board for disqualifying status at any time. This request could be made even prior to applying to a veterinary program or veterinary technology program or before applying for the Board’s CAET training course.
